ONE. Firstly, open up your image. heres the image I'm going to use :


TWO. Duplicate the image and go to Filter >> Blur >> Radial blur and set your settings to
Amount : 20 to 40
Blur Method : Spin
Quality : Good

THREE. Now use the eraser tool and set it to Size : 30-500px (this depends on how big or small your image is) and Hardness : 20% and erase over the face and body.
And your done, heres my result :
